   This work is a product, natural result, of a total of more than 50 years spent by the two editors teaching residents and other students the intricacies of surgical technique. As all who teach and learn and per form operations know, teaching surgical technique must be a labor of love because the intellectual into such endeavors incalculable. Literally thousands of surgeons have been technique by a like number of preceptors, since teaching technique in the opexperience. Because of the time and effort involved in teaching surgical skills, it was decided to take the format this work beyond that of an atlas that would simply the steps in operation. Instead, the format is directed toward showing the reader how to improve performance of procedures described by emphasizing the refinementsteve of technique developed by respective authors and which they, through personal experience, have found to work well. In some instances, the entire procedure has been described in detail; in others the focus is on prevention of common mistakes or guide nuances of the procedure that will save time or trauma to both patient and surgeon. Knowledge of important ana- tomic conceptsis a critical ingredient in surgical technique, and important areas have been highlighted in chapters at the beginning of most sections describing the pertinent anatomic landmarks and interelawe tionships useful to know performing certain procedures. Editor's comments are provided to empha size important points or, frequently, to add a contrapuntal view. In instance these comments intended criticize or fute the author's approach, but rather add a second view of possible strategies for a successful approach to surgical lem. In several instances, chapters the same topic are provided by authors to describe essentially frequently performed operation somewhat disparate points of emphasis as to highlight several effective complish the surgical objectives.
