 It would be well to begin this book with a brief description of its history. The first edition of Surgery of The Alimentary Trad was edited by Dr. Richard T. Shackelford and published in 1955. It rapidly established itself as an invaluable source of information to thousands of practicing general surgeons and residents in training during the following years. Owing to the book's success and the important contributions that it made, the W. B. Saun Company urged Dr. Shackelford to produce a second edition. It soon became obvious that because of the rapid growth and development of surgical techniques and new approaches to the treatment of disease, the second edition would have to be expanded substantially. Dr Shackelford completed the work on the first volume dealing with surgery of the esophagus and I became coeditor, responsible for the completion of the succeeding four volumes. This was accomplished with the help of several devoted faculty members at The Johns Hopkins Medical School, and the ranks of contributors ders were expanded to include individuals with cial expertise in other, newl total of five volumes and represented a significant step forward in the presentation of surgical science, while still including information of historic result, the second edition was almost totally new, but this was accomplished without departing from the approach that had been so successful in the first edition. ly developing, areas. The field of Gastrointestinal Surgery had continued to advance with major and important strides, building on the man advances in surgical research that had emerged during the preceding decade. Furthermore, for this edition, I enlisted the help of a guest editor for each volume of this series. As a r we were able to incorporate not only the fresh ideas that came from new authors but also in most cases new and contemporary illustrations We have now come to the fourth edition of this series. Once again this has been accomplished with the help of my colleagues who have served as guest editors for each of the five volumes The publication of these volumes is particularly timely for there have been dramatic changes in surgical practice during the past few years. We are fortunate to be able to present these innovations, together with the contributions of many new authors. Once again I would like to express my appreciation to the many individuals who have contributed chapters or sections to these volumes for their dedication and co contributors to the fourth edition are leaders in their fields, and I am deeply indebted to them for sharing in this commitment with great enthusiasm. I would also like to thank several individuals for their personal contributions to the enterprise. Lisette Bralow and Hazel N. Hacker of the W. B. Saunders Company have been major sources of inspiration and help during all phases of preparation of the books. I would also like to acknowledge the help of Barbara B.
