     The sixth edition of Principles of Surgery has special significance for the three editors G. Tom Shires, Frank C. Spencer, and me. As we reflect on all that has ensued since the time we met as a group with David Hume, Richard C. Lillehei, and Edward Storer in the summer of 1966 and accepted the challenge from McGraw-Hill to create a new textbook of surgery, many thoughts enter our minds. The extraordinary explosion of scientific information and the advancement of the field of surgery are apparent. We have attempted to incorporate the advances in each of the editions The extent of change and update is readily manifest. In Basic Considerations, six of the twelve chapters have been completely revised by new authors. These are chapters on Surgical Infections, Burns, Wound Care and Wound Healing, Oncology, Surgical Complications, and Physiologic Monitoring. And many other chapters incorporate significant changes. In the Specific Considerations section, the chapters on Breast, Tumors of the Head and Neck, Peripheral Arterial Disease, Manifestations of Gastrointestinal Disease, Esophagus and Diaphragmatic Hernia, Colon, Rectum, and Anus, Gallbladder and Biliary Tract, Pancreas, Abdominal Wall Hernias, Pituitary, Urology, Gynecology, Neurosurgery, and Orthopaedics all represent new presentations by new authors. The application of minimally invasive procedures is considered in each of these chapters. New data leading to new concepts comprise the essence of each chapter, whether the chapter is a replacement or a revision The sixth edition is the most major revision we have produced; this is a deliberate effort on our part because we realize we have been privileged to participate in the education of an entire generation of medical students, surgical residents, and practitioners. We are thankful to the many authors who over the years have contributed to the six editions of our book; it is the synthesis of their individual fields of expertise that has created a meaningful textbook. We are equally appreciative of our readership, both those readers who entrust their entrance into the arena of surgical education to us and those who have been loyal partners through several editions over the years.
